Practices will have to manually enter their achievements for the dementia enhanced services before 16 June in order to revalidate the claims.
The number of patients with a record of receiving a dementia assessment must be entered manually into CQRS for 2013/14.
If a practice fails to enter data before 16 June, an achievement of zero will be recorded and no payment will be made.
